Purpose: Revised OPAR Forms
To ensure foreign travel requests are processed more efficiently and accurately, the Travel Policy and Systems Branch (TPSB) has revised the Official Passport Action Request (OPAR) form. There are now two forms associated with processing foreign travel requests. For country clearance, visa, and amendment requests only, travelers are required to complete the OPAR A Form. To request issuance of a passport or to renew a passport, travelers are required to complete the OPAR B Form. When completing the OPAR forms, please ensure the required fields are completed.
In addition, the TPSB has provided written procedures to assist Travelers, Travel Specialists, and Program Support Assistants when submitting requests for the issuance or renewal of a passport.
