At this point, we are not collecting or maintaining the forms -- the individual is to keep it on their person while working in our buildings/worksites. \\ These visitors/contractors/collaborates who cannot attest to vaccination or will not provide such information must adhere to all restrictions, to include weekly or twice-weekly screening requirements.
